Introduce and attach (scan if necessary) the following three documents:

1) Your Internship Student Evaluation of Internship (at the bottom of this page). BRING A HARD COPY OF THIS COMPLETED FOR TO MEETING #6 for Ron to turn into the school.

2) Your Student KSA Self Assessment...show me your before and after scores on the same form. (Completed after the internship.)  

3) Your Portfolio Assessment. (Or see the attachment below.)  When the course is over, you can remove this document.
